Personal Data Privacy Notice

1. Controller and scope

Prof. Dr. Muhittin Taşkapılı is the data controller under Turkish Law No. 6698 (KVKK). Contact details appear on this page. This notice covers website use and contact/appointment requests. Separate information is provided for health records created during clinical care.

2. Data collected

The form collects your name, telephone number, preferred contact channel (telephone or WhatsApp), optional message and form language. A request reference and the displayed privacy notice version are also sent to Formspark.

When you submit the form, your browser connects to Formspark. Formspark and the hosting provider may process technical data such as IP address, browser information and submission time for connection, security and spam prevention purposes.

Do not include diagnoses, test results, reports, medication details, identity numbers or information about other people. This form is not a clinical assessment channel.

3. Purposes and legal grounds

Data necessary to assess an appointment request, respond and arrange an appointment is processed under Article 5(2)(c), where directly necessary for entering into or performing a contract.

Responses to general enquiries, request follow-up and system security rely on legitimate interests under Article 5(2)(f), subject to your fundamental rights. Article 5(2)(ç) applies to specific legal obligations and Article 5(2)(e) to establishing, exercising or defending a right, only where necessary.

The form does not subscribe you to advertising or marketing. This notice is not a consent request. Submitting the form does not constitute blanket consent to health-data processing or overseas transfers.

4. Recipients and overseas processing

Authorised clinic staff and, as necessary, hosting, technical support and email providers may process your request. Disclosure to public authorities occurs only where required by law.

The contact form is submitted directly to Formspark; notifications are sent to the email recipients configured for the form. Your name, telephone number, contact preference, message, language and technical form fields may be processed by Formspark and the email provider. These providers may process data outside Türkiye. Opening WhatsApp or social media links also involves the relevant providers.

Overseas transfers separately require the conditions in Article 9 of the KVKK. Displaying this notice or submitting the form does not itself establish a transfer safeguard or consent.

5. Collection and retention

Data is collected electronically through the form you complete and technical service records. Messages are held in the access-controlled Formspark account and email accounts receiving notifications. This deployment does not maintain an additional local message database.

Retention is limited to resolving and following up the request, applicable legal obligations and specific legal claims. Data must be deleted, destroyed or anonymised when the grounds for processing cease. You may contact us about retention or deletion.

6. Your rights

Under Article 11 you may ask whether your data is processed, request information, learn its purposes and whether use is consistent with them, learn domestic and overseas recipients, and request correction.

You may also request deletion or destruction under Article 7, notification of corrections/deletion to recipients, object to an adverse result based solely on automated analysis, and seek compensation for damage caused by unlawful processing.

7. Making a request

Submit a written request to the clinic address on this page, or email us using an address previously provided to and registered with the clinic. Other legally recognised application methods remain available. You may use “KVKK Request” as the subject.

Include enough information to verify your identity, your request and a reply address. Do not send unnecessary medical records or identity copies. Additional verification may be requested. Requests are normally answered free of charge as soon as possible and within 30 days; charges permitted by the Board’s tariff may apply if an additional cost arises.

8. Cookies and external services

A request reference is held in browser memory while the contact page remains open. This reference alone does not mean duplicate submissions are prevented by Formspark.

Google Maps, Google Fonts, YouTube players and Instagram videos you open connect to their respective providers and may transmit IP/browser information. Those services apply their own privacy and cookie practices. Linked social media and news websites are responsible for their own processing.
